Ultrasound Referral
The below referral form is for our vascular laboratory located in Nedlands. Please fill out the form and fax it back to us on (08) 9386 9866 or email through to reception@westcoastvascular.com.au, alternatively the patient call us directly to arrange an appointment.
All ultrasounds are bulk billed to Medicare or DVA.
Compression Stockings Referral
WestCoast Vascular supplies Sigvaris compression stockings to all patients. A referral is only necessary for DVA patients. Referrals can be faxed back on (08) 9386 9866 or emailed through to reception@westcoastvascular.com.au. All patients need to be measured for their compression stockings, this can be done in the Nedlands office or arranged externally.
